How to fill performance appraisal form for self appraisal

The following parameters to be rated by the employee for self appraisal.  The employee ratings and comments are reviewed by manager and based on that manager will give final rating to the employee.

1. Applied Creative Knowledge & Problem Solving Skills

  • Demonstrates working knowledge, subject matter of own role and is able to independently apply the concepts  to solve problems or achieve results.
  • Accurately assesses own scope of technical knowledge and knows when to get appropriate help.
  • Is proficient with the job processes and quality standards and is able to follow them under general direction

2. Building team value

  • Demonstrates value as a team player within the team
  • Takes bottom-line responsibility for his/her role in the team and the tasks assigned
  • Actively participates in team activities contributing ideas adding to the team's knowledge/ productivity
  • Mentoring team members
  • Communication with the team

3. Communication

  • Is able to articulate the concepts and procedures required to perform the job
  • Is able to explain problems and basic solutions to team members
  • Has adequate oral communication skills to contribute effectively in team meetings
  • Has adequate written communication skills to write documentation and conduct effective business interaction
  • Should be able to escalate things to the seniors / Management before crisis situation arises

4. Delivering Customer Value

  • Understands client's requirements well
  • Focuses energies on activities that directly impact the deliverables to the customer
  • Is aware of the quality standards and timeline required by the customer and works to maintain them

5. Operational & Procedural Management & Execution

  • Plans and sequences personal tasks to accomplish short and long term goals effectively
  • Follows established procedures efficiently to deliver quality results on time
  • Is knowledgeable about quality standards and follows them effectively

6. Continuous Learning

  • Seeks skill development when necessary to remain successful;
  • Keeps study of latest developments in job area;
  • Learns from experience, acts on feedback, and successfully improves performance;
  • Consistently meets improvement goals

7. Training attended

  • Relevant training/certification acquired
  • Relevant skills picked up during the appraisal period

8. Planning, Analysis & Organizing

  • Is accurate in forecasting conditions and comes up with methodical and realistic action plans for achievement of short term & long term objectives.
  • Demonstrates situational leadership & professional excellence in whatever he/she does by optimally utilizing the resources available

9. Results Orientation

  • Focuses on tasks or goals until satisfactorily completed
  • Demonstrates thoroughness, timeliness and accuracy of work output
  • Consistently achieves measurable outcomes

10. Initiative & Drive

  • Highly resourceful and a self-starter.
  • Generates quality ideas and provides useful suggestions

11. Leadership

  • Exhibiting the leadership qualities
